At your first appointment, you will be asked to fill out two forms: a New Patient Information Form and a Privacy Consent Form. You can fill these forms out at our office before your appointment or download them and fill them out in the comfort of your own home.

If you need help filling out these forms, just ask! The Tappuni Dental staff will be glad to walk through the forms with you.
